- the four segments into which the middle cerebral artery is divided, M1–M4 in clinical terminology. The M1 segment is the sphenoid part (pars sphenoidalis), the horizontal portion of the vessel from the termination of the internal carotid artery to the point of branching at the limen insulae; M2 is the insular part (pars insularis), the segment running in the lateral cerebral (sylvian) fissure and serving the insular cortex; M3 is the opercular part, the segment emerging from the sylvian fissure, supplying the inner aspects of the opercula, and ending at the lateral surface of the cerebral hemisphere; M4 is the cortical part, the terminal branches supplying the lateral surface of the cerebral hemisphere. The artery usually bifurcates into inferior and superior trunks at the end of the M1 segment, so there are two each of the segments M2–M4, the inferior distributing below the sylvian fissure, and the superior distributing above.
