- hyperglycemic hyperosmolar nonketotic state, hyperosmolar hyperglycemic state a metabolic derangement in which there is an abnormally high serum glucose level without ketoacidosis, causing fluid loss from cells to extracellular fluid. The cells become dehydrated, blood volume decreases, polyuria ensues, and some patients lapse into a coma (hyperglycemic hyperosmolar coma) that can be fatal. The condition occurs as a complication of borderline and unrecognized diabetes mellitus, in pancreatic disorders that interfere with production of insulin, as a complication of extensive burns, in conditions marked by an excess of steroids, or in acute stress conditions including infections. It also may develop during total parenteral nutrition, hemodialysis, or peritoneal dialysis.
